Description

The management of patients’ living with two or more chronic conditions (multi-morbidity) is an increasing challenge for 21st century clinicians. Multi-morbidity has been associated with reduced quality of life, functional decline, and increased use of health and social care services. This module will explore how health and social care systems can support patients with multiple chronic conditions. Students will learn about the impact of multi-morbidity on primary care, and explore patient and system-level approaches to managing multi-morbidity in primary care. Furthermore, students will meet an expert patient, with lived experience of the physical, and psycho-social aspects of multimorbidity.

The course critically evaluates the impact of the environment and social factors (social isolation, drugs and alcohol, social inequality) on health and multimorbidity. Leading researchers in the fields of: ageing and illness, social isolation and loneliness, polypharmacy, frailty, social prescribing, drugs and alcohol, and holistic medicine contribute to the teaching on this course.
